Work as a Behavioral Health Consultant embedded in primary care setting Provide evidence-based screenings, risk assessments, and brief interventions Collaborate with interdisciplinary team including psychiatrists, psychologists, and primary care providers Deliver whole-person, integrated care using biopsychosocial model Assist patients with accessing community mental health resources Provide care across the lifespan in a fast-paced clinical environment Participate in care coordination and consultation with medical staff Part of innovative, system-wide behavioral health integration program
