Full Job Description SUMMARY:
The Palliative Care Nurse Practitioner, under clinical supervision of a licensed physician, is responsible for diagnosis, treatment and management of patients who are dealing with a progressive illness and provides care in keeping with parameters established for nurse practitioners. As a primary care provider, the nurse practitioner will provide services appropriate for a hospice and palliative care specialist.
Essential job responsibilities:
Under the guidelines and protocols established for practice and specialty, performs patient histories, physicals and assessments developing a plan of care for symptom control and goals of care along the physical, psychological, emotional, social and spiritual domains of suffering, working alongside the interdisciplinary team. The palliative care nurse practitioner will collaborate with the covering physicians/medical as appropriate. The palliative care nurse practitioner may provide hospice and/or palliative care services in a variety of settings including hospitals, clinics, home care, and nursing homes and assisted living while consistently demonstrating the Hospice Savannah competencies, values, and service excellence standards. The palliative care nurse practitioner is accountable to and evaluated by the Steward Center medical director.
QUALIFICATIONS
Masters Degree in Nursing with successful completion of an accredited Nurse Practitioner Program. Board Certified as an Adult, Family, or Gerontology Nurse Practitioner. Certification in Hospice and Palliative Care preferred. Experience preferred in the care of terminally ill patients as defined by: three years nursing experience with demonstrated experience in the principles of pain and symptom management among palliative care populations. Ability to work successfully within an interdisciplinary team (including volunteers) toward positive outcomes for patients and their families. Ability to represent HSI within the physician community as well as general public with excellent communication and presentation skills. Requires basic computer skills in Microsoft Word products including Word and Outlook.
PHYSICAL DEMANDS
Position requires moderate to intense visual concentration, reading, using a computer and reviewing patient charts. This position has frequent contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers and HSI staff requiring excellent communications and problem solving skills and professional demeanor. There is a risk of exposure to infectious diseases, which may require use of standard precautions. The NP requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.
WORK ENVIRONMENT
Works a schedule as determined by supervisor. Face to face recertification visits in hospital, long-term care facility or home environment with patients. Rounds at the hospice inpatient unit (HIPU) Confidentiality required at all times.
