Nurse Practitioner - Internal Medicine for Primary Care

Job Summary Tanner Health is seeking a full-time Nurse Practitioner or Physician Assistant to join the Tanner Medical Group Bridge Clinic team which is a transitional care facility designed to "bridge "the gap and support patients during the critical period between hospital discharge and a follow-up visit with a primary care provider (PCP). Job Duties & Responsibilities Provide primary care services including medication management, chronic disease monitoring, coordination of care with specialists and patient education. Job Eligibility Requirements Education & Licensure Requires completion of an accredited Nurse Practitioner program, with a master's degree in nursing OR master's degree from an accredited Physician Assistant program Active GA APRN license OR GA Physician Assistant license or meet criteria for obtaining licensure. Must be board certified as a Physician Assistant. Experience Prior experience in adult medicine is preferred. New graduates are welcome to apply.

Schedule & Location:

Full-time, Monday

• Friday, 8am
• 5pm. Position will be located require shifts in Carrollton and Villa Rica Bridge Clinic locations. Area of Responsibilities

• Customer Service:

  Participates in patient satisfaction initiatives. Demonstrates excellent customer service skills. Communicates effectively with patients and management regarding service recovery opportunities. Works together with other team members to achieve Office and System customer service goals.

• Development:

  Maintain and update level of skill for pertinent medical assistant duties. Participates in continuing education. Participates in the development of new programs (ex: Process Improvement, Quality Initiatives).

Patient Care:

Performs evaluations, provide treatment and counsel patients. Order and interpret test results and recommend treatment of patients. Administer therapeutic procedures. Prescribe therapy or medication with physician approval. Provide physicians with assistance during surgery or complicated medical procedures. Demonstrates specialty and population specific competencies. Demonstrates an ability to effectively communicate based on specific specialty and population. Performs other duties as assigned by the supervising physician.

• Quality:

  Maintains evidence based medical care and clinical charting. Participates with clinical tracking and feedback. Integrates with clinical and administrative teams to administer high quality care.

Teamwork:

Works well with others and functions as a team player. Offers ideas and solutions for issues that affect the team and work area. Participates in the training and skills development of new clinical staff. Assists as needed.

• Current certification as a Nurse Practitioner in Georgia
• Graduate of a recognized program for Nurse Practitioner training (MSN).
• Must be capable of dealing tactfully and effectively with patients, family members, other employees and physicians
• Must maintain high degree of confidentiality.
• Must meet credentialing criteria.
• Must possess the cognitive ability to understand patient medical records, physician orders and medical orders to be communicated to patient, hospital and/or other physician.
• Must possess the sense or discipline to work in accordance with accepted clinical standards.
• Prior experience in a like specialty preferred.

Definitions

• Nurse practitioner provides patient services through the performance of patient evaluation, assessment and treatment in the medical office setting as allowed by state licensure laws under the supervision of the medical director.

The nurse practitioner works closely with the clinic staff, hospital departments and community agencies.
