Job Description

Job Overview Wound Care Hawaiʻi is seeking a dedicated Mobile Wound Care Nurse Practitioner (NP) to provide advanced wound care services to patients in their homes, skilled nursing facilities (SNFs), assisted living communities (ALFs), and other community-based settings across Oʻahu. This is a highly autonomous, on-the-road clinical role ideal for an NP who values independence, flexibility, and meaningful patient relationships. You will deliver evidence-based wound care directly at the bedside, helping patients heal in the comfort of their own environment while collaborating closely with our physician leadership and care coordination team. Compensation:
Competitive visit-based pay with earning potential exceeding $100+/hour equivalent (details discussed during interview) Key Responsibilities Perform comprehensive wound assessments and develop individualized treatment plans in mobile care settings Provide advanced wound care interventions (debridement, dressings, infection management, offloading, etc.) Deliver patient and caregiver education to promote healing and prevent complications Document care and track wound progress in EMR in accordance with HIPAA and regulatory standards Coordinate care with supervising physicians, home health agencies, facility staff, and families Travel between patient locations (homes, SNFs, ALFs, and community sites) across assigned territory Participate in care plan reviews and clinical collaboration with the Wound Care Hawaiʻi team Qualifications Licensed Nurse Practitioner (NP) in the State of Hawaiʻi Current BLS/CPR certification Clinical experience in wound care, geriatrics, home health, hospice/palliative, or post-acute care (preferred) Strong assessment and independent clinical decision-making skills Comfortable working autonomously in mobile/community settings Valid Hawaiʻi driver's license and reliable transportation required Ability to manage travel between patient locations throughout the day Why Join Wound Care Hawaiʻi Autonomy and flexibility in a mobile practice model Meaningful one-on-one patient care Supportive physician-led wound care team Growing, mission-driven local healthcare organization Opportunity to specialize and advance in wound care Work Location Mobile / Field-Based (Oʻahu) Patient homes, SNFs, ALFs, and community facilities

$144,000.00 - $177,000.00 per year
