Job Description

Job Overview We are seeking a dynamic and compassionate Wellness Nurse Practitioner to join our innovative healthcare team. In this vital role, you will deliver comprehensive primary and specialized care, focusing on promoting overall wellness, managing acute and chronic conditions, and supporting patients across diverse settings. Your expertise will empower individuals to achieve optimal health outcomes through personalized care plans, health coaching, and advanced clinical interventions. Duties Conduct thorough patient assessments including vital signs, physical examinations, and detailed medical histories to determine health status and needs. Develop and implement individualized care plans incorporating wellness strategies, medication management, and preventive care initiatives. Administer injections such as botulinum toxin treatments and dermal fillers, ensuring proper technique and patient safety. Manage acute pain cases, perform suturing, catheterizations, IV insertions, and other minor procedures with precision and aseptic technique. Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks for accurate documentation of patient encounters, treatments, and medical records. Provide telehealth consultations to expand access to care and deliver health coaching tailored to patient goals. Collaborate with multidisciplinary teams including specialists in gastroenterology, geriatrics, behavioral health, infectious disease care, hospice & palliative medicine, and more to coordinate comprehensive patient services. Participate in clinical research initiatives or quality improvement projects aimed at enhancing patient outcomes and operational efficiency. Educate patients on health maintenance, disease prevention, medication administration, nutrition, and lifestyle modifications. Support discharge planning, case management activities, and utilization review processes to optimize resource use and patient safety. Skills Proven experience with EMR/EHR systems such as Epic or Athenahealth; proficiency in CPT coding, ICD-9/10 coding is highly desirable. Strong clinical skills across various specialties including internal medicine, emergency medicine, ICU/trauma center experience (Level I & II), hospital medicine, outpatient clinics, nursing home care, assisted living facilities, home care settings, and pediatric or neonatal care. Expertise in performing advanced procedures such as spinal taps (lumbar punctures), sonography (medical imaging), gastric lavage, tube feeding management, catheterization techniques (urinary or vascular), sterile processing practices. Knowledge of infection control protocols and infection prevention strategies aligned with NCQA standards. Ability to manage complex cases involving acute pain management, occupational health issues including Workers' Compensation law compliance, hospice & palliative care principles. Familiarity with medical terminology related to cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), anesthesiology (airway management), psychopharmacology for behavioral health support. Experience working with diverse populations including individuals with developmental disabilities or dementia/cognitive impairments such as Alzheimer's or memory care needs. Excellent communication skills for patient education and team collaboration; ability to work effectively in high-pressure environments like emergency rooms or trauma centers. Physical ability to perform duties requiring standing for long periods; comfort with physical examinations and minor surgical procedures.
