No Position Overview PACE clinic coverage Nurse Practitioner preferred, Physician Assistant considered Maternity leave coverage Patient visits may range from 30 minutes to a few hours Census approximately 247 patients Patient population ages 55 and older Epic experience is helpful but not required Clinical Scope Comprehensive geriatric assessments Ongoing management of chronic conditions including diabetes, hypertension, COPD, and CHF Medication reconciliation and deprescribing Coordination of care with interdisciplinary team Annual wellness visits and acute care follow ups Advanced care planning and goals of care discussions Support Team Pharmacists Occupational and Physical Therapists Nurse case manager Triage nurse Medical assistants Community nurses LPN Required Qualifications IA license DEA BLS Medicaid and Medicare number required before presentation Credentialing/Onboarding Items (must be provided) Credentialing timeframe is typically 30 to 90 days Required credentialing documents as requested by the client team
