Position Overview We are seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team. The ideal candidate excels at building rapport, takes a person-centered approach to care, and communicates effectively across disciplines. You'll work in collaboration with a multidisciplinary team to deliver comprehensive psychiatric services in post-acute and long-term care settings. Key Responsibilities Conduct follow-up psychiatric visits after initial assessments Prescribe medications and review diagnostic results as needed Participate in team meetings and family sessions Collaborate with supervising psychiatrists and facility staff to implement treatment plans Maintain timely documentation and communication with care teams Provide education to facility staff regarding psychiatric care and patient needs Evaluate effectiveness of prescribed medications and make adjustments Ensure compliance with CMS guidelines and facility policies
