Opportunity:
Join a highly respected Internal Medicine team within a multi-specialty practice focused on adult and geriatric care Full-time position with a strong emphasis on work-life balance Compensation range: $95,280
  • 144,976 Practice with clinical autonomy while supported by robust administrative and operational resources Focus on adult and geriatric populations, managing chronic conditions and preventive health Light 1:16 call schedule (telephone only) Collaborative team environment with experienced Internists and Nurse Practitioners Supportive, mission-driven culture with long-tenured providers and strong community ties
Responsibilities:
Provide comprehensive care to adult and geriatric patients in an outpatient setting Conduct detailed health histories, physical examinations, and risk assessments Diagnose and manage acute and chronic medical conditions Develop and implement individualized treatment and care plans Order, interpret, and follow up on diagnostic tests and lab results Prescribe medications and therapies in accordance with state regulations Coordinate care with specialists and other members of the healthcare team Promote preventive care, wellness, and chronic disease management Educate patients and families on health conditions and treatment plans Maintain accurate and timely documentation within the
EMR Qualifications:
Master's Degree or Doctorate (DNP) in Nursing from an accredited institution Certification as a Nurse Practitioner (FNP or AGNP) Strong clinical and critical-thinking skills with the ability to work independently and collaboratively Excellent communication and interpersonal skills Commitment to compassionate, patient-centered, evidence-based care Interest in practicing in a community-based or rural healthcare setting
Licensure/Certifications:
Current Illinois APRN license (or eligibility) DEA license (or ability to obtain)
Compensation:
