We provide exceptional serious illness care, hospice care and grief support for adults and children. The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
Nurse Practitioner-Field:
The Field Nurse Practitioner is an advanced practice nurse who provides comprehensive hospice care under physician supervision. Responsibilities include assessing patients, diagnosing conditions, developing care plans, prescribing treatments, and evaluating hospice eligibility. The role collaborates with patients, families, and the interdisciplinary team to deliver care in home and community settings.
Key Responsibilities:
Conduct comprehensive patient assessments, including medical history, physical exams, and hospice eligibility evaluations. Develop and manage individualized plans of care, including prescribing medications and coordinating treatments. Perform face-to-face visits for hospice certification and recertification in compliance with regulatory requirements. Collaborate with physicians, the interdisciplinary team, and community providers to ensure coordinated patient care. Monitor and manage symptoms to ensure patient comfort and quality of life. Maintain accurate, timely documentation of clinical assessments, visits, and care plans. Educate and support patients, families, and caregivers on care needs, disease progression, and end-of-life care. Participate in IDT meetings and contribute to ongoing care planning and quality improvement initiatives. Primarily supports the southern team Occasional travel to other areas as needed Monday through Friday, 8:00 am to 4:30 pm schedule
Education/Experience:
Graduate of an accredited Nursing and Nurse Practitioner program Current state RN/NP licensure Master's degree with 1+ year NP experience and 1+ year hospice/palliative experience Certified by an approved advanced practice nursing body by state Specialty certification preferred (e.g., Hospice/Palliative, Pediatrics, Geriatrics) Valid driver's license and insured, reliable vehicle
