We are a multi-specialty healthcare team dedicated to providing personalized and seamless patient care. Through established partnerships, we offer individualized access to comprehensive medical services, including examinations, postoperative follow-up, and ongoing care. Our goal is to eliminate frustration and ensure peace of mind for every patient. Key Responsibilities Perform and document patient evaluations within the hospital setting, ensuring effective coordination with nursing staff, ancillary providers, attending physicians, administration, and other medical and patient care personnel. Communicate clearly and compassionately with patients and their families regarding care plans and treatment progress.Work schedule:
Monday-Friday, with coverage every third weekend. Hybrid model with potential nursing home coverage included. No overnight call required. No procedural duties are required. No Code or Rapid Response Team (RRT) responsibilities. No ICU coverage required. Manage admissions for 2-4 patients per shift. Average daily rounding on 15-20 patients.Paid time off:
15 days, plus 5 days for CME (Continuing Medical Education). Qualifications Licensed and nationally certified Nurse Practitioner (e.g., AGACNP or FNP) with demonstrated expertise in hospital-based care, including acute medical management, care transitions, and team-based rounding. Current and unrestricted NP license to practice in Illinois, with valid DEA and Controlled Substance registration. Clinical experience in hospital medicine required.
