Part-Time Nurse Practitioner: Annual Wellness Visits

Multiple Locations (Northern, Southern or Western suburbs The Role This is a part-time, multi-site clinical position tailored for a Nurse Practitioner who excels in geriatric assessment and preventative care. In this role, you will lead Annual Wellness Visits (AWVs) in your region, ensuring our geriatric population receives comprehensive, high-quality screenings and personalized prevention plans.

You will:

Conduct comprehensive Annual Wellness Visits at various Assisted Living, Independent Living, and Long-term Care facilities. Perform detailed cognitive screenings, functional assessments, and fall risk evaluations. Lead medication reconciliation and facilitate meaningful Goals-of-Care discussions. Develop and update patient-centered Care Plans based on AWV findings to address chronic conditions and preventative health goals. Identify and enroll eligible patients into Chronic Care Management (CCM) programs, ensuring they receive the continuous support necessary for complex health needs. Ensure thorough documentation and clinical coding to capture all wellness measures What Makes This Role Unique Specialized Clinical Focus AWVs provide patients with a no-cost, dedicated visit focused entirely on prevention and early detection. $75 per hour/completed visit
