Job Overview We are seeking a dedicated and highly skilled Family Nurse Practitioner (FNP) to join our dynamic healthcare team. The ideal candidate will possess a comprehensive understanding of primary care, acute and chronic disease management, and a passion for delivering exceptional patient-centered care. This role involves providing comprehensive health assessments, managing diverse patient populations, and collaborating with multidisciplinary teams to promote wellness across all age groups. Duties Conduct thorough physical examinations, health assessments, and diagnostic evaluations for patients of all ages, including pediatrics and geriatrics. Develop personalized treatment plans, prescribe medications, and administer injections or immunizations in accordance with clinical guidelines. Manage acute illnesses, chronic conditions, and perform procedures such as suturing, IV infusions, catheterizations, and basic life support interventions. Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks to document patient encounters accurately and efficiently. Provide health coaching, education on disease prevention, family planning, and post-acute discharge planning to support overall wellness. Collaborate with specialists in areas such as gastroenterology, infectious disease care, behavioral health, hospice & palliative medicine, and senior care to ensure comprehensive patient management. Participate in clinical research initiatives or quality improvement projects aimed at enhancing patient outcomes and adhering to NCQA standards. Skills Proven experience working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities, and those requiring specialized care such as hospice or memory care. Strong proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth; familiarity with CPT coding, ICD-10/ICD-9 coding standards is essential. Extensive knowledge of clinical procedures such as phlebotomy, venipuncture, sterile processing techniques, gastroscopy preparations like gastric lavage or spinal taps. Ability to perform advanced assessments involving physiology knowledge, diagnostic evaluation techniques like sonography or laboratory testing. Experience managing acute pain cases using modalities such as botulinum toxin treatment or esthetic laser treatments is advantageous. Excellent communication skills for triage calls via telehealth platforms; adept at case management and utilization review processes. Knowledge of occupational health protocols including Workers' Compensation law; familiarity with hospital medicine environments such as Level I & II trauma centers or ICU settings enhances versatility. Commitment to patient safety through infection control practices; understanding HIPAA regulations for medical documentation and patient confidentiality. $120,000.00 - $150,000.00 per year
