In this position, you will leverage your advanced clinical skills and broad medical knowledge to assess, diagnose, and treat patients primarily elderly, ensuring comprehensive care that promotes health and well-being. Your expertise will support our mission to provide accessible, high-quality healthcare services through innovative practices and compassionate patient interactions. This paid position offers an exciting opportunity to make a meaningful impact in a fast-paced, collaborative environment. Duties Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories. Develop and implement individualized treatment plans utilizing evidence-based practices across specialties such as primary care, urgent care, geriatrics, and behavioral health. Utilize electronic health record (EHR) systems such as AthenaHealth for accurate charting and medical records management. Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to optimize patient outcomes. Provide telehealth consultations and triage services to extend care access beyond traditional settings. Support specialized areas such as hospice & palliative medicine. Experience Proven clinical experience in diverse healthcare environments including hospital medicine, emergency medicine, ICU/CCU/PICU settings, nursing homes, assisted living facilities, or outpatient clinics. Demonstrated proficiency with EMR/EHR like Athenahealth for comprehensive documentation and patient management. Familiarity with CPT coding, ICD coding (ICD-9/10), and utilization management enhances your ability to navigate billing and compliance processes efficiently. Background working with special populations including geriatrics (memory/dementia care), or patients with developmental disabilities is advantageous. Experience administering injections, performing phlebotomy or venipuncture with sterile technique while maintaining patient safety standards. Exposure to medical imaging techniques or physical therapy support like Hoyer lifts can add value to your role.
