This position is best suited for an experienced and highly independent provider who is comfortable managing medically complex adult patients, communicating with interdisciplinary teams, and working closely with collaborating physicians. Our organization focuses on delivering strong clinical care, responsive communication, and long-term facility partnerships in the post-acute space. What We Offer Full-time position Physician-led and operationally supported environment High level of autonomy with physician collaboration Long-term growth opportunity within a growing organization Responsibilities Daily rounding and follow-up care Perform admissions, progress notes, discharge planning, and coordination of care Communicate with physicians, nursing staff, therapists, case management, and families Manage acute and chronic medical conditions in post-acute settings Complete timely and accurate documentation Preferred Experience Hospitalist or Internal Medicine experience strongly preferred Experience in inpatient post-acute care, LTAC or rehab hospital settings preferred Comfortable working independently and making clinical decisions Strong communication and bedside manner Ability to manage patient volume efficiently while maintaining quality care Qualifications Active Indiana NP license APRN / Nurse Practitioner certification DEA license preferred Master's or Doctorate in Nursing preferred Compensation Competitive compensation package Full-time consistent schedule Supportive clinical and operational infrastructure If you are looking for an autonomous internal medicine role with strong physician support and a stable long-term opportunity in the Indianapolis market, we would love to connect.
