In this vital role, you will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. Your expertise will help promote health, prevent illness, and manage acute and chronic conditions in various clinical settings. This position offers an exciting opportunity to make a meaningful impact on individuals' lives through expert clinical care, patient education, and collaborative teamwork. You will utilize advanced EMR systems and stay current with evolving medical practices to deliver exceptional patient-centered care. Duties Conduct thorough physical examinations, assess patient health status, and develop personalized treatment plans. Manage a wide range of conditions including acute illnesses, chronic diseases, behavioral health issues, and geriatric concerns. Administer injections, perform suturing, IV infusions, catheterizations, and other procedures with precision and sterile technique. Utilize EMR systems such as Epic or eClinicalWorks for accurate documentation of patient encounters and medical records. Provide health coaching, patient education on disease prevention, medication management, and lifestyle modifications. Collaborate with specialists in areas such as gastroenterology, infectious disease care, hospice & palliative medicine, and occupational health to ensure comprehensive care delivery. Participate in telehealth consultations and triage services to extend access to quality healthcare remotely. Assist in diagnostic evaluations using sonography or laboratory specimen collection/processing when necessary. Stay compliant with HIPAA regulations while maintaining confidentiality of patient information. Contribute to discharge planning, case management, utilization review, and post-acute care coordination for complex cases. Experience Valid Family Nurse Practitioner license with current certification from an accredited program. Demonstrated experience working in diverse clinical environments such as hospital medicine, emergency medicine, ICU or trauma centers (Level I or II trauma experience preferred). Proven proficiency with EMR/EHR systems including Epic, Cerner or Athenahealth; telehealth experience is a plus. Strong background in managing acute pain, infection control protocols, wound suturing, and IV insertion techniques. Experience working with vulnerable populations such as assisted living residents, nursing home patients or individuals with developmental disabilities. Familiarity with CPT coding and ICD coding (ICD-9/10) for accurate documentation and billing processes. Knowledge of Workers' Compensation law and utilization management procedures is advantageous. Prior involvement in clinical research or teaching hospital settings is desirable but not required.
