Home - Fortis About the Role We are looking for a compassionate Nurse Practitioner to join our hospice team. This role is ideal for someone dedicated to providing comfort, dignity, and high-quality palliative care to patients and their families during life's most meaningful moments. If you are collaborative, patient-centered, and passionate about hospice care, we would love to connect with you.
Current positions available:
PRN Part-Time Full-Time Responsibilities Conduct required face-to-face hospice visits and report findings to the Hospice Medical Director Develop, coordinate, and document patient plans of care Collaborate with attending physicians and IDG members Prescribe medications (including controlled substances) within scope and delegation Order diagnostics and treatments as appropriate Educate patients, families, staff, and community on hospice and palliative care Participate in IDG and QAPI meetings Maintain accurate clinical documentation for compliance and reimbursement Supervise and evaluate home health aides as appropriate Complete required training modules
