Job Description

We are looking for a dedicated Nurse Practitioner with at least 1 year of patient care experience to join our team. We are a sole proprietorship, locally owned PCP office that services patients from newborn to geriatric. Position requires the ability to treat both acute and chronic illnesses. Must be able to provide quality care with little supervision. We work closely with local home health agencies, to maintain a high standard of care for our patient who are homebound. Active KY licensure and unrestricted DEA are required. Seeking Part-Time position, but Full-Time and Part-Time applicants will be considered. We currently have 1 MD, and 1 APRN in our office. You must be willing to be credentialed with all of our current accepted insurances, including Medicare and Medicaid. We assist in the credentialing process. Duties include but are not limited to: provide primary care to patients in our clinic, reviewing medical history, medication lists, developing and documenting a care plan as well as contributing to existing care plans, following acute and chronic illnesses/diseases, perform physical exams, annual Medicare exams, screening and patient observations, order and analyze diagnostic studies/tests, diagnose and treat health issues, evaluate responses to treatments and medications, educate patients and families on managing illnesses or injuries and disease prevention, follow and order preventative care testing and immunizations, recommend further treatment option or referrals and future appointments when needed, work closely with our nursing and front office staff to properly care for our patient population. We are not able to offer health insurance at this time but do offer 401K matching with applicable position.
