Care2U is currently seeking a part-time (with potential for full-time) Adult Gerontology Nurse Practitioner / Family Nurse Practitioner to work in the Owensboro & Madisonville, KY area. Position Summary The Adult Gerontology Nurse Practitioner (AGNP) / Family Nurse Practitioner plays a critical role in Care2U's mobile care model by delivering high-quality primary care services directly to patients in community-based settings. This includes senior living communities, group homes and other mobile practice homes & facilities. This role is designed for a provider who values flexibility, autonomy, and mission-driven work—someone who wants to practice medicine differently by meeting patients where they are and improving access to care. Key Responsibilities Conduct in-person primary care visits, including evaluations, follow-ups, and chronic care management Assess, diagnose, and treat acute and chronic medical conditions Develop and manage individualized care plans based on patient needs Prescribe medications and manage ongoing treatment plans in accordance with best practices Provide patient and caregiver education to support long-term health outcomes Collaborate with Care2U providers, pharmacy teams, and facility staff to ensure coordinated care Complete timely, accurate, and compliant documentation within the EMR Maintain all licensure, certifications, and prescribing authority required to practice Represent Care2U with professionalism while living out our mission and values in the community Qualifications Current and unrestricted Nurse Practitioner license Board certification as a Adult Gerontology Nurse Practitioner (AGNP) / Family Nurse Practitioner DEA registration and prescriptive authority in good standing or can obtain Strong clinical decision-making skills and ability to work independently Excellent communication and relationship-building abilities Experience in primary care, geriatrics, or community-based/mobile care preferred Willingness to travel within assigned service areas Physical Demands Sit, Stand and walk for extended periods (4+ hour shifts) Lift or move 10-15 pounds occasionally Frequent bending, reaching, and repositioning Ability to respond quickly in emergency situations Work in high-stress, fast-paced environments Why Join Care2U Flexible, part-time schedule with potential to develop into full-time Mission-driven work with meaningful patient impact Autonomy in a mobile care setting Collaborative and supportive team environment Mileage reimbursement Opportunity to be part of an innovative and growing model of care Apply We are looking for a provider who is clinically strong, compassionate, and motivated to make a real difference in patient access and outcomes.
