Job Description

Job Type Full-time Description FLSA Status:
Exempt I.
General Areas of Responsibility:
Provides outpatient clinical management for the patients of PFH's community health center, who span from pediatrics to geriatrics, as well as its GBT Wellness Center for gay and bisexual men and transgender people. II.
Detailed Areas of Responsibility:
Practices in collaboration with a licensed physician (or physicians) pursuant to a collaborative practice agreement. Enhances the quality of care through education and advanced nursing knowledge and practice. Effectively collaborates with the interdisciplinary care team. Provides diagnosis, treatment, consultation and follow-up to patients. Orders and interprets tests as indicated and recommends subsequent treatment for patients. Administers therapeutic procedures; prescribes therapy or medications within defined scope of practice. Participates in organizational quality improvement initiatives. III.
Compliance & Quality Assurance Standards:
Must cooperate with the organization's Quality Assurance and Improvement Program
  • includes actively participating in all required trainings and in creating a culture of quality improvement
  • Cooperation with the quality program will be considered for performance reviews. Must cooperate with the organization's Compliance Practices
  • includes immediately reporting issues, actively participating in required trainings, following HIPAA regulations, and assisting in creating a culture of compliance.
Also includes compliance with written standards and all applicable federal and state laws. Cooperation with compliance will be considered for performance evaluations. Not currently listed as excluded or sanctioned by the Department of Health and Human Services, Office of Inspector General (OIG), the General Services Administration (GSA), or excluded from participation in any federal program. Agrees to promptly notify supervisor in the event of any such debarment, exclusion, conviction, threat, or indictment ocurring during the term of employement. Failure to comply with the organization's Compliance Practices, which include failure to report conduct or events that potentially violate any laws or the organization's written compliance standards will be met by disciplinary action, up to and including possible termination. Chief Medical Officer, Chief Executive Officer, and/or any other management-level employee designated by the Chief Executive Officer. V.
Methods of Accountability:
Interface with Chief Medical Officer, Chief Executive Officer, and other PFH managers. Individual management meetings as scheduled. Periodic Performance Reviews VI. Staff Directly Co-Supervised with
Director of Clinical Services and Director of Quality Services:
Clinic Nurses (LPNs) Certified Medical Assistants (MAs) Requirements VII.
Qualifications:
Ability to think proactively and function independently. Excellent interpersonal skills and ability to work effectively with others in the workplace. Expertise using Microsoft Office software suite required. Must possess a strong understanding of HIPAA regulations. Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals across the organization. Must be detail oriented and able to manage complex projects and multiple tasks. Must maintain high level of professionalism and ensure confidentiality. Treat clients, staff, volunteers, board members, etc. with dignity and respect. Offer assistance to coworkers as needed. Demonstrate initiative by actively participating in the resolution of problems. Ask for assistance when learning new information or job duties. Take all necessary measures to properly use and maintan supplies and equipment. Respect the rights of coworkers to a productive workplace. Must adhere to all practices specified in the Employee Handbook and organizational Operating Procedures. VIII. Education and/or
Experience:
Graduate of an accredited school of nursing Graduate of an accredited Nurse Practitioner Program Current Advanced Practice Registered Nurse (APRN) Licensure Current Nurse Practitioner certification in specialty area Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Strong computer skills, including
Word, Excel, and PowerPoint Experience:
A minimum of one year of experience is preferred. IX.
Physical Demands:
While performing the duties of this job, associate is regularly required to sit, stand, and walk. Carry supplies and other materials as needed during the normal course of work. X.
Work Environment:
Medical clinic environment On-call for irregular lab notices on rotating schedule. XI.
Other:
