Family Nurse Practitioner

A well-established community health organization in Hyannis, MA is seeking a Family Nurse Practitioner to join its primary care team. This full-time, outpatient position offers the opportunity to provide comprehensive care to a diverse patient population, including routine wellness visits, chronic disease management, and acute care. The ideal candidate will have at least 2 years of experience , the ability to work independently, and a commitment to serving underserved communities. This role offers a collaborative, team-based environment , where providers work alongside RNs, MAs, and interpreters to ensure high-quality, patient-centered care. The schedule is Monday through Friday , with one evening shift per week and a rotating Saturday schedule based on clinic needs. The clinic utilizes Epic EMR and has a strong support system, including on-site behavioral health and care coordination services. This is an excellent opportunity for an experienced FNP looking for a stable, mission-driven role in a Federally Qualified Health Center (FQHC) setting.
