Key Responsibilities Conduct in-depth health assessments and develop individualized care plans Provide holistic medical care, addressing physical, mental, and social needs Coordinate with Behavioral Specialists, Care Coordinators, and nursing staff Assist patients with non-medical needs such as securing housing, employment, or transportation Maintain patient engagement without volume-based pressure-no set daily patient quota Qualifications Minimum of 3 years of clinical experience, including primary care Experience in integrated care or harm reduction models preferred Comfort working with high-risk populations (PTSD, severe anxiety, SUD, disabilities) Strong collaboration, communication, and problem-solving skills Support and Environment Extensive multidisciplinary support team including Behavioral Health Specialists and Care Coordinators Robust nursing support and administrative resources Freedom to shape workflows, workflows, and patient-centered protocols Why Pittsfield, MA?
