Johns Hopkins Hospital is seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at the Heart Failure Bridge Clinic (HFBC) in the Levi Watson, Jr. Outpatient Center. The selected candidate will coordinate the daily activities of the HFBC, manage complex patient cases, and ensure adherence to established protocols in both clinical and research settings. This role offers the opportunity to work directly with faculty in the Advanced Heart Failure program.Schedule:
Full-time, 40 hours No nights, no weekends.Responsibilities:
Clinical Care Assess and treat adult heart failure patients referred to the Heart Failure Bridge Clinic (HFBC). Manage complex cases, including determining appropriate treatment plans and coordinating with attending physicians. Perform assessments of volume status and decide on IV diuretic treatment when needed. Develop patient-specific care plans and communicate them to the Registered Nurse (RN) for implementation. Act as a clinical resource to the HFBC RN and Medical Assistant (MA). Approve patient transfers, determine discharge appropriateness, and coordinate follow-up care. Bill for clinical care time spent with patients. Coordination & Liaison Act as a liaison between the Heart Failure Bridge Clinic, specialty Heart Failure outpatient programs, and inpatient teams. Ensure that the clinic follows established protocols and meets clinical and research standards. Coordinate daily clinic activities, ensuring smooth operations and high-quality patient care.Required Qualifications:
Adult Gerontology Nurse Practitioner, or Acute Care Nurse Practitioner (ACNP) certification is accepted. Must be licensed to practice as a Nurse Practitioner or Physician Assistant in the state of Maryland. Prior experience in cardiology is required. Experience in heart failure management is preferred. graduates considered when accompanied by prior cardiology nursing experience or adequate prior clinical exposure/rotation in cardiology.Additional Information:
This position will require working with a dynamic team in a specialized cardiology clinic. Autonomous practice and independent clinical decision-making. Minimum 53.15/hour - Maximum 82.38/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
