Own your panel. Practice at the top of your license. Autonomous, field-based work — Monday through Friday. What You'll Do Manage a dedicated SNF patient panel with routine and acute visits Assess, diagnose, and treat chronic conditions and geriatric syndromes Lead care transitions — admissions, readmissions, and discharges Conduct Health Risk Assessments and build individualized care plans Collaborate with RNs, care coordinators, and facility staff Compensation & Perks $135,000
