Job Description

JOB SUMMARY
The HIV Nurse Practitioner provides direction to the organization with respect to HIV medical care. In addition to serving as the primary clinical consultant in HIV care, this provider keeps staff clinicians abreast of developments in HIV medicine and actively pursues opportunities to further develop and improve the HIV care that CBHS provides. This provider for HIV Clinical Care collaborates closely with the Lead Physician for HIV Care to provide unified leadership in Infectious Disease. This position represents CBHS to the medical and patient communities, as well as the community at-large, as an expert in HIV Medicine
MAJOR DUTIES AND RESPONSIBILITIES
TeamworkProvides direction to the organization with respect to HIV issuesFunctions as CBHS primary clinical consultant in HIVCollaborates with Pharmacy staff in maximizing an HIV formulary and assisting the pharmacy in seeking inclusions of HIV agents in MCO formularies as they become FDA approved CommunicationKeeps CBHS clinical staff aware of updates to HIV guidelines and new research findings through regular presentations at staff meetingsEnsures that medical staff and other interested/involved departments (nursing, CMOD) are aware of current clinical trials and enrollment criteriaRepresents CBHS to the medical and patient Communities, as well as the community at-large, as an expert in HIV medicineReads and responds to e-mail in a timely mannerAnswers phone calls/pages within 30 minutesServes on MST Task ForceResponds promptly to queries from sponsors about specific casesParticipates in internal committees and teams as requestedIs an activist for his/her patients (data sources: peer review, chart audits) Advocates for referral to MST for HIV patients starting ARVs or changing ARVs Analytical and Critical ThinkingApplies the Care Model to patient careMaintains the standard of clinical care, using diagnostic testing appropriately yet Cost-effective, and providing appropriate treatmentAssesses for barriers and assets in a patients social environment that are likely to affect health outcomesProblem Solving and Decision MakingReviews research patients labs within 2 working days of receipt and takes appropriate action if abnormalPatient FocusEffectively engages patients in their care. Makes sure that patient calls are returned (by self or other staff) within 2 workingdaysAddresses patient concerns brought up during visit, and documents assessment and plan for these concernsProfessionalismDemonstrates professionalism and integrity in all aspects of patient care Workplace Computer and EquipmentMaintains complete, legible, and comprehensive documentationUtilizes EMR resources to manage patient care, flags, protocols, referralsServes on MST Task Force and advocates for referral to MST for all HIV patients starting a new medication regimen or undergoing a medication regimen change.

Checking, Examining & RecordingConsistently completes progress notes of patient encounters by end of next business dayConsistently removes inactive medications from patients med listsConsistently removes inactive medications from patients problem listsIs timely with respect to completion of necessary paperwork such as disability formsCompletes paperwork and billing in a timely and complete mannerCompletes clinic billing and coding documentation in a complete and accurate mannerDocuments social history (ever) in 90% of patients seen at least 3 times as PCP.Allergies are documented (ever) in 90% of patients seen at least 3 times as PCP.Health habits and risk factors are documented (ever) in 90% of patients seen at least 3 times as PCP.Sexual history is updated within the last two years in 50% of cases audited.

Immunizations are up to date in at least 50% of patients reviewed in chart auditSelf-management goal has been documented/updated within one year for patients with chronic disease at least 40% of time in chart audit Compliance, Policies and ProceduresKeeps current with respect to organizational policies and procedures, and puts them into action accordinglyShows meaningful progress toward their quality improvement goalsInterpersonal SkillsIs approachable to the nurse or midlevel provider on call.
SKILLS AND ABILITIES
Ability to work with a multidisciplinary clinical teamComputer skills including Microsoft office suite, electronic medical records and practice management system
EDUCATION AND/OR EXPERIENCE
Required:
NP with completion of a one-year residency training program with focus on HIV Care, and training in a combination of internal medicine, family practice and experience working with HIV/AIDS and Infectious Diseases.
Desired:
AAHIVM certification.
Required:
Minimum 1-year experience in treating people living with
HIV/AIDS
in a ethnically, culturally, and racially diverse environment.

Experience working in a non-profit healthcare environmentCurrent Maryland License to practice medicineFederal DEA licenseLGBTQ candidates are highly desired.
