Job Description

Job Overview We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our dedicated healthcare team. In this vital role, you will provide comprehensive primary care services across diverse patient populations, including pediatrics, geriatrics, and adult patients. Your expertise will support patients through preventive care, acute illness management, chronic disease treatment, and health education. Duties Conduct thorough physical examinations and patient assessments to determine health status and develop personalized care plans. Manage acute and chronic conditions utilizing evidence-based practices, including medication administration, wound care, suturing, and IV infusion therapy. Provide patient education on disease prevention, health promotion, medication adherence, and lifestyle modifications. Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks to document clinical encounters accurately and efficiently. Perform specialized procedures including dermal fillers, botulinum toxin treatments, spinal taps, and esthetic laser treatments when qualified. Collaborate with multidisciplinary teams to coordinate care for patients in settings such as assisted living facilities, nursing homes, hospitals, or outpatient clinics. Engage in telehealth consultations to extend access to care and support remote patient management. Requirements Valid Family Nurse Practitioner certification/license with current state registration. Proven experience in primary care settings with familiarity in medical office procedures and documentation standards. Demonstrated proficiency with EMR/EHR systems like Epic or Athenahealth for efficient patient record management. Strong clinical skills including physical examinations, diagnostic evaluation, specimen collection/processing, and medication administration. Experience working with diverse populations such as pediatrics, geriatrics, patients with developmental disabilities or dementia care needs. Knowledge of CPT coding, ICD-10 coding systems for billing and documentation purposes. Critical care experience or hospital medicine background preferred; experience in emergency medicine or ICU is a plus. Skills in triage assessment, acute pain management, infection control practices, and infection disease care are highly desirable. Ability to perform procedures such as suturing, catheterization, phlebotomy, venipuncture, and sterile processing techniques. Familiarity with Workers' Compensation law and hospice & palliative medicine principles is advantageous. Join us to deliver exceptional patient-centered care that improves lives!
