Our model emphasizes thoughtful, relationship-driven medicine with a strong focus on chronic disease management, prevention, and long-term patient outcomes within a collaborative, team-based environment. We are seeking a Nurse Practitioner who enjoys managing complex adult patients in an outpatient setting and is passionate about delivering high-quality, patient-centered care. Experience or interest in endocrinology, diabetes, obesity medicine, or metabolic health is strongly welcomed, though not required. Position Overview The NP will work closely with physicians and the broader clinical team to manage patients with a wide range of chronic medical conditions, including cardiometabolic and endocrine disorders. This role offers the opportunity to build meaningful long-term patient relationships while practicing in a technology-forward environment designed to support high-quality care and reduce administrative burden. Clinical Focus Areas Chronic disease management Internal medicine and preventative care Diabetes management (Type 1 & Type 2) Obesity and metabolic health Hypertension and cardiovascular risk reduction Thyroid and endocrine disorders (nice to have) Longitudinal outpatient care Ideal Candidate Licensed Nurse Practitioner (NP) Background in internal medicine, endocrinology, or chronic disease management preferred Strong interest in adult medicine and longitudinal outpatient care Experience with diabetes, metabolic health, or endocrinology is a plus but not required Collaborative and patient-centered approach to care Excellent communication and relationship-building skills Comfortable working in a fast-growing outpatient environment Why Join NCMI?
