About Us:
Synergy Medical Group provides comprehensive, patient-centered specialty care across multiple disciplines. Our Medicine division is expanding in Salisbury, MD, and we are seeking a dedicated Medicine Nurse Practitioner (NP) to join our growing post-acute care team. At Synergy, we pride ourselves on a collaborative, multidisciplinary approach that fosters clinical excellence and supports professional development across skilled nursing facilities, assisted living communities, and chronic disease management settings.
Position Overview:
The Medicine NP plays an essential role in delivering high-quality primary and acute-care services within skilled nursing facilities (SNFs) and post-acute care environments in the Salisbury region. This role involves comprehensive medical management, coordination of care, and ensuring continuity for patients with complex medical needs. Providers should be comfortable practicing autonomously while collaborating with attending physicians, nursing leadership, and interdisciplinary teams.
Responsibilities:
Perform comprehensive history and physical examinations for SNF and post-acute patients Diagnose and manage acute and chronic medical conditions (e.g., diabetes, CHF, COPD, hypertension, post-surgical recovery, infections, dementia-related medical issues) Order, review, and interpret diagnostic tests including labs, imaging, and specialty consults Develop, implement, and modify individualized treatment plans Manage transitions of care, including admissions, follow-ups, and discharges Coordinate care with attending physicians, nursing staff, therapy teams, and case management Provide patient and family education regarding medical conditions and care expectations Ensure timely and accurate EMR documentation Participate in IDT (Interdisciplinary Team) meetings as needed
Qualifications:
Licensed Nurse Practitioner in Maryland Board Certified FNP, AGNP, or ACNP Skilled Nursing Facility (SNF) or post-acute care experience strongly preferred Internal medicine, geriatrics, or primary care background desirable DEA license or eligibility to obtain Strong clinical assessment, communication, and decision-making skills Commitment to patient-centered care and collaborative practice
