Advanced Practice Providers, NP, or PA - Mission-Driven, Team-Based Care in Maine Join Penobscot Community Health Care, a highly respected Federally Qualified Health Center (FQHC) in Maine. Treat the whole person with comprehensive, patient-centered care. Choose from PCHC's outpatient primary care locations in Bangor, Brewer, Old Town, Belfast, or Jackman. Our fully integrated care model lets you focus on delivering high-quality care while working alongside a collaborative, multidisciplinary team on-site and across the system. PCHC's services include behavioral health, dental care, primary care pharmacy, rehabilitation, substance use disorder treatment, case management, chronic disease support, and more—all designed to improve outcomes and strengthen the communities we serve. Physicians here serve not only as caregivers, but also as leaders and advocates who advance population health and support underserved communities.
