Collaborate closely with physicians, nurses, social workers, community health workers, and settlement partners to coordinate care and system navigation for clients with complex social and medical needs. Participate in case conferences, outreach clinics at Scarborough shelters and community sites, and inter-agency initiatives addressing gaps in care for uninsured and refugee populations. Contribute to quality improvement, data collection, evaluation activities, and student supervision as appropriate to support CCRIHC's teaching and advocacy mandate.
