Job Description

As a Nurse Practitioner / Physical Assistant with Matrix, you'll deliver meaningful, one-on-one care during in-home Comprehensive Health Assessments (CHAs). These visits—typically up to one hour—allow you to see the full picture of a patient's health, far beyond what's possible in a traditional clinical setting. You'll have the time, autonomy, and support to make a real difference—helping identify risks, close care gaps, and guide patients toward better health outcomes. Self Scheduling Visits
  • Day, Evening, Weekend $95 per visit $1,500 Bonus What You'll Do Perform adult and geriatric health assessments, including medical history, diagnosis, treatment, and education Identify health risks and provide referrals for follow-up care, case management, or specialists Conduct point-of-care and lab-based testing as appropriate Collaborate with physicians, care teams, and caregivers to ensure coordinated care Partner with Primary Care Providers to support ongoing patient care and education Deliver care in patients' homes or skilled nursing facilities Why Providers Love Working at Matrix Flexibility That Fits Your Life You control your schedule.
With availability from 8 AM-7 PM, seven days a week, you can work when it suits you—days, evenings, or weekends. Strong Earning Potential Earn per completed assessment, with the opportunity to increase income through expanded availability and travel. Providers completing 20-100 visits per month typically earn between $1,800 and $11,500 monthly . Qualifications:
Master's Degree (or equivalent) and Nurse Practitioner / Physical Assistant licensure Active NP / PA license with prescriptive authority (or ability to obtain) Board certification (AANP, AACN, or ANCC) in an applicable specialty Current BLS, ACLS, or CPR certification 1+ year NP / PA experience preferred (new grads welcome!)
