You will work closely with committed physicians who prioritize mentorship, professional development, and team-based learning. This role offers an opportunity to collaborate with staff members to support seamless patient care. You will make a meaningful impact serving as a trusted partner to patients and families navigating health care needs. Coordinates and delivers primary care in collaboration with the Practice Site Physician by: Working within their approved scope of practice. Obtaining complete medical history and physical data on patients. Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed. Performing a complete physical exam and recording findings. Collaborating with physicians in managing the medical needs of patients. Order appropriate laboratory and diagnostic procedures. Educating and advising patients regarding health and illness prevention. Recommending community resources to meet patient and family needs. Support patient access and care coordination through triage, follow-up, and management of urgent situations Provides clinical and administrative assistance to BMG programs, e.g., education, follow-up, and the like by: Participating in health education. Maintaining related records and statistical data and preparing statistical reports as needed. Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors. Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes. Organizational Responsibilities Associate complies with the following organizational requirements: Attends and participates in department meetings and is accountable for all information shared. Completes mandatory education, annual competencies and department specific education within established timeframes. Completes annual employee health requirements within established timeframes. Maintains license/certification, registration in good standing throughout the fiscal year. Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department. Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patients and self. Adheres to regulatory agency requirements, survey process and compliance. Complies with established organization and department policies. Available to work overtime in addition to working additional or other shifts and schedules when required. Education/ Licensure / Certification Required Credential(s): Graduate of an accredited Physician Assistant or Nurse Practitioner program. Current, valid state licensure and appropriate certification (NCCPA or
ANCC/AANP
) obtained prior to hire date or job transfer date. Verified by local Credentialing Office. BLS obtained within 1 Month (30 days) of hire date or job transfer date if required. American Heart Association or American Red Cross accepted. Current BLS and ACLS certifications if required. Ability to work collaboratively in a fast-paced, team-oriented environment.
Knowledge & Skills:
Requires a thorough knowledge of practice standards and policies and procedures of BMG and the appropriate professional organization. Requires a high level of analytical skills necessary to assess the patient and participate in follow-up care planning. Demonstrates interpersonal skills necessary to effectively interact with patient(s), family of patient(s), physicians, other professionals and paraprofessionals. Demonstrates high level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.
Working Environment and Physical Demands:
Works in a clinic environment requiring physical exertion, frequent changes in job demands, and exposure to biohazards. Requires the physical ability and stamina (i.e. to walk/stand for prolonged periods of time, push carts/wheelchairs, lift up to 50 pounds, to position/lift patients at a maximum of 35 pounds without assistance, provide CPR, etc.) to perform the essential functions of the position.
