Earn up to $135K + Corporate Bonus—daytime NP role, no weekends or call.

$123,000-$135,000 base + corporate bonus

Mon-Fri, days • No on-call • No weekends
Team:
Skilled nursing & long-term care partners What You'll Do Provide care in nursing and long-term care facilities Manage chronic conditions and perform full assessments Close care gaps (HEDIS and STAR measures) Lead care planning with facility teams Support and educate patients' families Must-Haves Active, unrestricted APN license in Michigan Board certified and Medicare credentialed 6+ months in skilled nursing or long-term care Solid with digital tools and team-based care Able to work onsite in
