Job Description
Job Summary We are seeking a highly skilled and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will possess extensive clinical expertise across various specialties, including acute and chronic care, geriatrics, and behavioral health. This role involves providing comprehensive patient assessments, developing treatment plans, and delivering high-quality care in diverse settings such as outpatient clinics. Experience with advanced EMR/EHR systems like Epic, Cerner is preferred. The Nurse Practitioner will play a vital role in patient education, medication management, and coordination of care across multidisciplinary teams. Duties Conduct thorough physical examinations and patient assessments, including vital signs and medical history documentation using EMR/EHR systems such as Epic and RXNT Manage acute and chronic conditions across various specialties including geriatrics, internal medicine, and behavioral health Perform procedures such as suturing as needed Administer injections and medications following sterile techniques while adhering to HIPAA regulations and safety protocols Develop and implement individualized treatment plans based on diagnostic evaluations including laboratory tests, medical imaging, sonography, and diagnostic evaluation tools Provide telehealth consultations utilizing telemedicine platforms with experience in telehealth systems Coordinate discharge planning, case management, utilization review, and post-acute care planning for diverse patient populations including hospice & palliative medicine patients Educate patients on health coaching topics such as nutrition, family planning, memory care (Alzheimer's & dementia), senior care (including nursing home experience), pediatric & neonatal care (including neonatal ventilator management), and working with individuals with disabilities or developmental delays Maintain accurate medical documentation of patient encounters using EMR/EHR systems like RXNT or Cerner while ensuring compliance with NCQA standards Participate in clinical research projects or quality improvement initiatives related to infection control, utilization management, or medical imaging procedures Qualifications Valid Nurse Practitioner license with current state certification; additional certifications in acute pain management or hospice & palliative medicine are a plus Demonstrated experience in diverse clinical settings such as hospitals (including Level I & II trauma centers), outpatient clinics, nursing homes, assisted living facilities, or home care environments Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth; telehealth platforms; CPT coding;
ICD-9/10
coding; medical documentation standards Skills in performing advanced procedures such as suturing, IV insertion/infusion, catheterization (including Foley catheters), spinal taps; familiarity with dermal fillers and Botox treatments preferred Strong knowledge of physiology and anatomy relevant to primary care and specialized fields like cardiology (cardiac catheterization), ophthalmology (ophthalmology procedures), or infectious disease management Experience working with vulnerable populations including children (pediatrics/toddler/neonatal), elderly patients (geriatrics/memory/dementia care), individuals with disabilities or developmental delays Ability to work independently while collaborating effectively within multidisciplinary teams; excellent communication skills for patient education and discharge planning Certification in Basic Life Support (BLS) required; Advanced Cardiovascular Life Support (ACLS) preferred This position offers an opportunity to make a meaningful impact across a broad spectrum of patient needs while working within a dynamic healthcare environment that values expertise in multiple specialties.Job Types:
