Position Summary:

We are seeking a Wound Care focused Nurse Practitioner to provide mobile, on-site care across senior living communities . In this role, you will manage complex wounds, support chronic condition management, and serve as a key clinical partner within each community. This position offers high autonomy, flexible scheduling, and the ability to build long-term patient relationships while practicing at the top of your license.

Responsibilities:

Provide on-site rounding, primary care, and wound care services . Manage advanced wound cases including pressure ulcers, diabetic wounds, and skin integrity issues . Drive preventative care and chronic disease management strategies . Collaborate with physicians, nursing staff, and interdisciplinary care teams . Reduce hospitalizations through early intervention and consistent follow-up . Communicate with families and caregivers to align on care plans . Maintain accurate documentation, medication management, and lab coordination .

Qualifications:

Active Certified Nurse Practitioner (CNP) license in Minnesota (AGNP preferred) . Experience in geriatrics, wound care, primary care, or post-acute settings preferred . Valid driver's license and ability to travel between communities. Strong ability to work independently in a field-based environment . Passion for delivering high-quality care to the senior population . Military Occupational Classification (MOC)
