As part of your practice, you'll manage medically complex patients, collaborate closely with interdisciplinary care teams, and help improve outcomes through proactive intervention and ongoing clinical oversight. You'll also develop expertise in advanced wound care while serving as a valuable resource for nursing teams and community leadership. What You'll Be Doing Conduct routine provider rounding visits throughout your assigned territory Perform advanced wound assessments, treatment planning, and ongoing wound management Manage chronic conditions and medically complex patient populations Collaborate with physicians, nurses, therapists, caregivers, and facility leadership Support continuity of care and care transition initiatives Identify clinical concerns early and help reduce avoidable hospitalizations Provide patient and family education Build trusted relationships throughout the communities you serve Participate in quality improvement and specialty clinical initiatives Ideal Backgrounds We welcome Nurse Practitioners with experience in: Wound Care Geriatrics Primary Care Family Practice Internal Medicine Skilled Nursing Facilities Long-Term Care Post-Acute Care Home Health Hospice & Palliative Care Qualifications Active Minnesota Nurse Practitioner License in good standing Strong clinical assessment and decision-making skills Experience in wound care or interest in developing wound care expertise preferred Comfortable practicing independently while collaborating with physicians and interdisciplinary teams Passion for serving older adults and medically complex populations Valid driver's license and reliable transportation required Build a Practice That Leaves a Lasting Impact The best providers do more than diagnose and treat.
