Job Description

Job Overview Fairview is seeking a Nurse Practitioner to join our primary care practice at M Health Fairview Clinics - Burnsville. The Nurse Practitioner provides comprehensive, full-scope care to an assigned patient panel, including management of acute and chronic conditions, preventive care, and in-office procedures, as appropriate. Practice Details:
15-provider primary care practice Full scope patient panel Average of 18 patients per day FTE = 34 patient contact hours per week
Openings:
0.75 to 1.0 FTE (Full Time) - Flexible schedule As part of Fairview's Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management. Fully integrated EPIC electronic medical record Multidisciplinary team-based care model M Health Fairview Clinic - Burnsville is adjacent to Fairview Ridges Hospital.
Primary Responsibilities:
Assess, diagnose, and manage acute and chronic conditions Perform histories and physical exams Order and interpret diagnostics Develop and manage care plans Prescribe medications Patient education and communication Documentation and follow-up Collaboration with care team Referrals and care coordination

Qualifications:
Graduate of an accredited school of nursing and Nurse Practitioner training program. Board certified or board eligible as a Nurse Practitioner through AANP or ANCC Licensed or eligible for licensure as a Registered Nurse and Advanced Practice Registered Nurse (APRN) in Minnesota. Ability to obtain and maintain DEA certification in the state of Minnesota. Must meet credentialing and privileging requiremens. New grads welcome and encouraged to apply. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions withthe same min/max are flat rate jobs based on 40 hours/week. Compensation plans based on production can allow for higher pay than the range posted.
