Job Description

Job Summary This is not your average NP position. As the dedicated provider for three long-term care facilities in the area, you will serve as the primary clinical presence for a geriatric resident population — building the kind of meaningful, longitudinal relationships that define exceptional long-term care. You will be the provider these residents count on, and your clinical leadership will directly shape the quality of their daily lives. What You'll Do You will provide comprehensive primary care assessments, diagnoses, and treatment plans for geriatric residents across all three facilities, serving as the consistent clinical lead for each community. Day-to-day responsibilities include conducting thorough physical examinations, managing acute and chronic conditions common to the geriatric population, coordinating with multidisciplinary teams on discharge planning, case management, and utilization review, ensuring accurate and compliant documentation in EMR systems in accordance with HIPAA regulations, leading infection control initiatives and overseeing medication management including injections and IV infusions, implementing quality improvement strategies to enhance patient safety and satisfaction across all three sites, and providing clinical mentorship and guidance to facility staff to ensure adherence to protocols and quality standards. What You'll Need Active NP license in good standing in the state of practice Board certification by a national certifying body Experience working with geriatric and elderly patient populations — required Proficiency in EMR/EHR systems — Epic, Cerner, Athenahealth, or eClinicalWorks Strong knowledge of CPT and ICD-10 coding and medical documentation best practices Experience with chronic disease management, dementia care, and end-of-life transitions Familiarity with long-term care regulatory standards and compliance requirements Excellent communication and care coordination skills Telehealth experience a plus Hospice and palliative care experience a plus What's In It For You Competitive salary Comprehensive benefits package The opportunity to make a lasting and meaningful impact on the lives of long-term care residents across three facilities A collaborative, supportive work environment Professional growth and development opportunities
Pay:
$150,000.00 - $200,000.00 per year
