The practice offers a competitive compensation package The NP serves as a critical member of the care team and will provide direct patient care, coordinate care transitions, and help lead quality improvement efforts working in a behavioral health setting. The focus of this position is to provide high quality, cost-effective care to elderly patients suffering from Psychiatric issues and co-morbidities with a special emphasis on managing post-acute issues to prevent hospital readmissions. About Practice The practice providers work exclusively in a behavioral health setting to improve clinical outcomes, reduce re-admissions, and enhance the operational performance of the facility. Through our consistent presence and intensive focus on optimizing patient outcomes, our mission is to become the clinical partner of choice at the facilities in which we serve. Job Responsibilities Within scope of practice, effectively manages acute and chronic conditions by developing and overseeing high quality, cost-effective treatment plans for patients in the facility, including ordering and interpreting diagnostic tests and prescribing medications and other treatments as appropriate. Provides patient|family education and counseling in the areas of self-care, health promotion, maintenance, and disease prevention. Works closely with collaborating physicians and other colleagues, which allows for consultation, collaboration, and|or referral. Provides clinical leadership and education to nursing and other staff, including to facility performance improvement efforts. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations according to established standards in GCP's provider-friendly electronic health record. Analyzes new knowledge gained from conferences, workshops, professional literature, or hands-on training and assimilates this knowledge into clinical practice. Maintains strict confidentiality with regard to protected health information and understands and adheres to GCP's HIPAA Privacy and Security policy. Share in taking call with other GCP providers. Qualifications and Skills Current valid and unrestricted RN and NP licensure in Missouri. National Board Certification and MSN in related field strongly preferred. Experience in Acute care, or inpatient rehabilitation or psychiatric practice settings. Demonstrated interpersonal skills to be approachable and understandable to patients, families, and staff. Ability to be flexible to meet the practice schedule so as to ensure appropriate coverage and high quality patient care. Collaborative practice style and ability to work harmoniously with others. Demonstrated skill in resolving conflicts and promoting positive outcomes for patients and families.
