ASAM Level 3.7 - Medically Monitored Residential Withdrawal Management ASAM Level 3.5 - Clinically Managed High-Intensity Residential Services Position Summary The Nurse Practitioner (NP) provides medical assessment, treatment, and clinical oversight for residents receiving care in an ASAM Level 3.7 and ASAM Level 3.5 residential substance use disorder treatment program. The NP works collaboratively with the Medical Director, Director of Nursing, and Clinical Team to ensure the delivery of safe, evidence-based, and trauma-informed care for adults with substance use and co-occurring mental health disorders. Under the supervision of the Medical Director, the NP conducts medical and evaluations, prescribes and monitors medications, supports withdrawal management, and ensures compliance with Missouri Department of Mental Health (DMH) regulations, ASAM criteria, and applicable federal standards. This role helps establish the medical infrastructure of a growing residential treatment program focused on integrated, patient-centered recovery. Additionally, the Nurse Practitioner should demonstrate competency and clinical comfort in managing common medical conditions and medications that may arise in the residential treatment setting. This includes, but is not limited to, the administration and monitoring of sliding scale insulin, management of basic antihypertensive medications, and other routine medical treatments necessary to support patient health and safety." Essential Duties and Responsibilities Medical Assessment and Treatment Conduct comprehensive medical evaluations upon admission and throughout treatment. Develop and implement individualized medical care plans consistent with ASAM Level 3.7 and 3.5 standards. Assess and manage acute and chronic health conditions, withdrawal symptoms, and co-occurring psychiatric disorders. Prescribe and monitor medications in accordance with state and federal law, including Medication-Assisted Treatment (MAT) when appropriate. Order, review, and interpret laboratory, diagnostic, and toxicology results. Coordinate referrals to specialty providers or hospitals when necessary. Provide client education regarding medications, chronic disease management, and preventive health. Support nursing staff in implementing medical protocols and ensuring readiness for medical emergencies. Collaboration and Clinical Oversight Collaborate with the Medical Director, Director of Nursing, Clinical Supervisor, and interdisciplinary treatment team. Participate in treatment team meetings to review client progress and adjust medical treatment plans. Communicate with external healthcare providers, pharmacies, and hospitals to ensure continuity of care. Provide clinical consultation and guidance to nursing and clinical staff regarding withdrawal management, infection control, and medication safety. Participate in on-call rotation to support 24-hour clinical coverage. Compliance and Documentation Maintain accurate, complete, and timely documentation within the Electronic Medical Record (EMR). Ensure compliance with Missouri Department of Mental Health (DMH), ASAM Criteria, HIPAA, 42 CFR Part 2, and DEA regulations. Participate in quality improvement initiatives, medical chart reviews, and regulatory audits. Assist leadership in preparation for state licensing inspections and accreditation surveys. Client Education and Recovery Support Provide education to clients and families regarding health literacy, medication adherence, relapse prevention, and chronic disease management. Assist clients in understanding treatment goals and discharge care plans. Model trauma-informed, culturally responsive, and recovery-oriented care in all client interactions. Safety and Risk Management Monitor infection control practices, universal precautions, and medical emergency protocols. Respond to medical incidents or client crises according to facility policy. Maintain professional competency and required licensure through continuing education. The Psychiatric Mental Health Nurse Practitioner (PMHNP) and/or Advanced Practice Registered Nurse (APRN) is responsible for the appropriate use of the Missouri Prescription Drug Monitoring Program (PDMP) in accordance with state regulations and organizational policies.Key responsibilities include:
Access and Review:
Access the PDMP as an authorized prescriber to review patients' controlled substance prescription histories prior to initiating and periodically throughout the course of treatment involving scheduled medications.Clinical Decision-Making:
Utilize PDMP data to inform safe prescribing practices, identify potential misuse, diversion, or polypharmacy risks, and support appropriate treatment planning.Ongoing Monitoring:
Routinely monitor patient activity within the PDMP for individuals receiving controlled substances, ensuring continued appropriateness of prescribed medications.Documentation:
Document PDMP reviews and any clinically relevant findings in the patient's medical record in accordance with organizational standards.Compliance:
Ensure all access and use of the PDMP is compliant with applicable state laws, including maintaining secure login credentials and accessing records solely for legitimate patient care purposes.Collaboration:
Communicate relevant PDMP findings with interdisciplinary team members, including nursing and pharmacy staff, to support coordinated patient care. Delegation Oversight (if applicable): Supervise and take responsibility for any authorized delegates accessing the PDMP on their behalf, ensuring proper registration and appropriate use.Education and Best Practices:
Stay informed of current regulations, guidelines, and best practices related to controlled substance prescribing and PDMP utilization. This responsibility is essential to promoting patient safety, reducing the risk of medication misuse, and ensuring compliance with regulatory standards.Minimum Qualifications Education:
Master's or Doctorate in Nursing from an accredited institution.Licensure:
Active Nurse Practitioner license in the State of Missouri in good standing.Certifications:
Current DEA registration for prescribing controlled substances.Experience:
Minimum two (2) years of clinical experience, preferably in behavioral health, detoxification programs, residential treatment, or integrated care settings. Skills and Knowledge Strong working knowledge of ASAM Level 3.5 and 3.7 criteria. Knowledge of withdrawal management protocols and co-occurring disorder treatment. Strong clinical assessment, diagnostic, and treatment planning skills. Excellent communication and interdisciplinary collaboration abilities. Ability to pass background check and drug screening in accordance with DMH requirements. Preferred Qualifications Experience in residential treatment, detoxification, or inpatient behavioral health programs. Familiarity with MAT protocols including buprenorphine and naltrexone. Training in trauma-informed care and motivational interviewing. Experience supervising or collaborating with nursing and allied health professionals. Schedule Full-time or part-time positions available. Scheduling may include flexible hours and participation in on-call medical coverage to support 24-hour residential operations.
