Homedica is seeking a compassionate and skilled Nurse Practitioner to deliver comprehensive primary care services across a variety of settings—including in-home visits, assisted living and nursing facilities, post-acute care hospitals, and via telehealth. Responsibilities:
Provide primary medical care services in home, facility, and telehealth settings Conduct physical exams, wellness visits, and manage chronic and acute conditions Order and interpret diagnostic tests and labs Prescribe and manage medications and individualized care plans Coordinate care with interdisciplinary teams and community partners Promote preventive care, patient education, and continuity of care Participate in after-hours or on-call rotation as needed
Qualifications:
Active Nurse Practitioner license in Louisiana DEA license required Experience in home health, hospice, or facility-based care required Strong communication, decision-making, and clinical skills Comfortable working independently across diverse care environments
