Job Description

Description Staff III - Psychiatric Nurse Practitioner - LMC Reports to Director of Behavioral Health The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides comprehensive psychiatric evaluation, diagnosis, and treatment to patients across the lifespan in an FQHC setting. The PMHNP delivers high-quality, patient-centered mental health care to underserved and medically vulnerable populations, working collaboratively within an integrated primary care and behavioral health team. Services are provided in accordance with FQHC, HRSA, and organizational standards, with an emphasis on access, equity, and continuity of care.
MINIMUM QUALIFICATIONS
Graduate degree from an accredited Psychiatric-Mental Health Nurse Practitioner program Current APRN licensure in good standing National certification as a PMHNP (ANCC or equivalent) DEA registration and prescriptive authority Eligibility for credentialing with Medicaid, Medicare, and other payers Minimum of three years' experience in the delivery of healthcare programs.
ESSENTIAL DUTIES
Conduct psychiatric assessments, diagnostic evaluations, and screenings Diagnose and treat mental health disorders using evidence-based practices Prescribe and manage psychotropic medications in accordance with state and federal regulations Provide brief therapeutic interventions and medication management visits Develop, implement, and monitor individualized treatment plans Manage patients with co-occurring medical, psychiatric, and substance use disorders Respond to psychiatric crises and coordinate appropriate levels of care Work collaboratively with primary care providers, behavioral health clinicians, nurses, case managers, dentists, allied health staff and support staff Participate in integrated care models such as collaborative care or team-based treatment planning Coordinate referrals to specialty mental health, substance use treatment, and community resources Consult with medical and behavioral health colleagues regarding patient care Maintain accurate, timely documentation in the electronic health record (EHR) Ensure compliance with FQHC, HRSA, and organizational policies Participate in quality improvement initiatives and clinical audits Meet productivity, quality, and access benchmarks Provide culturally competent, trauma-informed, and recovery-oriented care Serve patients regardless of ability to pay, using a sliding fee scale Support patient education, self-management, and engagement in treatment Address social determinants of health in care planning Maintain licensure, certification, and credentialing requirements Participate in staff meetings, trainings, and continuing education Contribute to program development and expansion of behavioral health services Support the mission, vision, and values of the health center Leadership by example and ability to communicate appropriate needs with administration and staff. Outreach to the community, area providers, hospitals, and clinics as needed for patient referrals in accordance with FQHC standards. Provide care on mobile unit, health fairs, schools and other businesses as needed. Other duties as assigned by management.
