Job Description

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our growing team of healthcare professionals serving Skilled Nursing Facilities (SNFs) and Long-Term Care (LTC) centers. The provider will deliver expert wound management, collaborate with interdisciplinary teams, and support our mission of improving patient outcomes through evidence-based wound care. This is an excellent opportunity for a provider interested in a focused practice area with predictable hours, a supportive team environment, and a patient-centered approach to care. Responsibilities Perform comprehensive wound assessments, develop individualized treatment plans, and provide ongoing wound care management. Collaborate with nursing staff to ensure high-quality patient care. Document patient encounters accurately, timely and in compliance with CMS and commercial payer guidelines. Educate facility staff on wound prevention, early identification, and treatment practices. Ensure compliance with
CMS, SNF/LTC
regulations, and facility policies. Serve as a clinical resource for advanced wound management, including debridement, negative pressure wound therapy, biologics, and advanced dressings. Qualifications NP or PA with an active, unrestricted medical license in the state of practice. Procedural experience preferred, but we often hire providers from Family Medicine, Internal Medicine, Geriatrics, or related specialty. Wound Care Certification (or willingness to obtain) preferred. Experience in Skilled Nursing, Long-Term Care, or Geriatrics is a bonus. Strong communication skills and ability to collaborate with patients and facility staff. Compensation & Benefits We offer a competitive compensation model designed to reward quality and efficiency: Uncapped Fee-For-Service compensation plan.
Annual Earning Potential:
$120,000-$150,000 (full-time equivalent). Malpractice insurance coverage. Mileage reimbursement.
