DESCRIPTION
Northwest Community Health Center (NWCHC) is looking for a full-time Advanced Practice Provider; Family Nurse Practitioner or Certified Physician Assistant, to join its multidisciplinary primary care team. NWCHC is an integrated primary care, behavioral health, pharmacy, and oral health delivery system in Libby and Troy Montana. The selected candidate will play a critical role in delivering a broad range of high-quality, compassionate primary care to patients of all ages including chronic disease management and follow up, acute care, as well as many in office procedures. He or she will be responsible for managing the needs and care of a distinct panel of patients in an outpatient setting. All NWCHC staff participate in quality assurance projects and work among highly qualified professionals in setting goals and improving access to healthcare for all. Successful applicants must be flexible with strong interpersonal skills and sound judgment, must maintain a current PA-C or Nurse Practitioner license in the State of Montana, and a unrestricted DEA registration. Currently, the organization is an approved NHSC site with a competitive HPSA score qualifying for loan repayment.
