TREK Healthcare is seeking a compassionate and experienced Part-Time Nurse Practitioner to join our growing team. This position will provide care to palliative care patients throughout the Missoula and Bitterroot Valley areas and offers a flexible schedule of 20-30 hours per week. The role provides a high level of autonomy and the opportunity to develop meaningful relationships with patients and families while making a lasting impact on their quality of life. The Nurse Practitioner will collaborate closely with physicians, nurses, and interdisciplinary team members to provide high-quality, patient-centered care in home and community settings. Position Details Part-Time Flexible Schedule Primarily Monday-Friday Travel within the service area required Competitive compensation based on experience Mileage reimbursement provided Essential Responsibilities Conduct comprehensive patient assessments and physical examinations. Evaluate, diagnose, and manage acute and chronic health conditions within scope of practice. Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans. Order, perform, and interpret diagnostic tests and laboratory results. Prescribe medications and treatments in accordance with Montana law and organizational policies. Provide symptom management and palliative care recommendations for hospice patients. Participate in care conferences and interdisciplinary team meetings. Educate patients, caregivers, and families regarding diagnoses, treatment plans, medications, and disease management. Complete accurate and timely clinical documentation in the electronic medical record. Ensure compliance with federal, state, and accreditation standards.
Qualifications Required:
Current Montana Nurse Practitioner license in good standing. Current DEA registration and prescriptive authority. Master's or Doctorate degree from an accredited Nurse Practitioner program. Current BLS certification. Valid driver's license and reliable transportation.
Preferred:
Previous Home Health, Hospice, Palliative Care, Primary Care, or Geriatric experience. Experience working independently in community-based healthcare settings. Knowledge of Medicare Conditions of Participation and hospice regulations.
