Job Description

A physician-led medical group is seeking Post-Acute Care Physicians to provide care in skilled nursing facilities and other post-acute settings. There are multiple openings in various locations across North Carolina. These positions offer a high level of independence, a flexible practice model, and the chance to build lasting relationships with patients, their families, and facility teams. It is a strong fit for a physician looking for meaningful clinical work, better work-life balance, and long-term growth. In this role, you would provide both primary and ongoing care to short-term rehab patients as well as long-term residents in post-acute facilities. Duties include evaluating patients, managing chronic conditions, diagnosing and treating acute and ongoing medical issues, developing and revising individualized care plans, ordering and interpreting lab work, prescribing and monitoring medications, collaborating with interdisciplinary care teams, and educating patients and families about treatment plans, prevention, and disease management. Maintaining timely and accurate documentation is also a key part of the role. The group offers solid clinical and operational support, along with onboarding, training, and opportunities for leadership and career advancement within a growing post-acute care organization. Qualifications Active medical license No unresolved disciplinary issues affecting licensure Interest in working in post-acute or long-term care settings Physicians who are not board certified are encouraged to apply.
