Provide comprehensive, patient-centered primary care to frail and elderly populations
Shift:
Days, full-time daytime schedule
Procedures:
Comprehensive participant assessments, care plan management
Patient Care:
Focused on geriatric and frail populations
Key Requirements Licensure:
Active NP License in
North Carolina Certifications:
Board Certified NP (ANCC or AANP),
DEA, CDS, BLS Experience:
Minimum 5-7 years NP experience, preferably in long-term care
Skills:
Experience with frail/elderly populations
Vaccination:
Required On-Call:
Participation in provider on-call rotation
