Senior Health Clinic Advanced Practice Provider Whitecap Search - 4.1 Fayetteville, NC Job Details $100,000 - $140,000 a year 1 day ago Qualifications Advanced practice provider Cross-functional communication Full Job Description A reputable health system in central North Carolina is seeking an experienced Physician Assistant or Nurse Practitioner to join their team, providing compassionate, high-quality care to the geriatric population. This is a mission-driven role focused on improving outcomes and quality of life for aging adults in both clinic and long-term care settings.
Job Function:
Provide care for geriatric patients in both clinic and affiliated nursing home settings Manage chronic conditions and support preventive care initiatives Participate in a collaborative, multidisciplinary care team Develop long-term patient relationships with a focus on continuity of care Deliver care aligned with quality and outcome-driven clinical standards
Job Requirements:
Licensed Physician Assistant (PA) or Nurse Practitioner (NP) in North Carolina (or eligible) 1-2+ years of experience in geriatrics, skilled nursing facilities (SNF), long-term care, or related settings Strong clinical judgment and patient-centered approach Excellent communication and teamwork skills Passion for providing high-quality care to the senior population
