The ideal candidate will provide high-quality patient care in family care homes, independent living facilities and homes of residents. This role requires a strong focus on patient assessment, diagnosis, and treatment planning, with an emphasis on delivering exceptional care to the geriatric population Responsibilities Conduct comprehensive patient assessments and develop individualized care plans to in home patients. Diagnose and manage acute and chronic health conditions. Provide urgent care services, including pain management and emergency interventions as needed. Collaborate with interdisciplinary teams to ensure continuity of care for patients. Utilize electronic health record systems for accurate documentation and patient management. Educate staff, patients and families on health maintenance and disease prevention strategies. Stay current with medical practices and protocols, including familiarity with ICD-9 coding for billing purposes. Qualifications Master's degree in Nursing or a related field from an accredited program. Current Nurse Practitioner licensure in the state of practice. Previous experience in inpatient/outpatient settings, or urgent care preferred. Experience working with adult population Knowledge of pain management techniques is a plus. Familiarity with electronic health record systems. Strong clinical assessment skills and the ability to make sound decisions under pressure. Excellent communication skills to effectively interact with patients, families, and healthcare team members.
