Summary This position is eligible for the Education Debt Reduction Program (EDRP)
a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval
award amount (up to $200
000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application.

seeking to create a personalized health plan for each Veteran This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being The Advanced Practice Nurse (APN) Ill Certified Nurse Practitioner (NP) practices in Primary Care Service to provide a full range of services emphasizing quality•timeliness•and the advancement of the Veterans Affairs (VA) mission The NP provides advanced clinical practice•consultation•and management Duties include: Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice Develops and implements an individualized plan to achieve optimal population health and disease management Demonstrates leadership and works collaboratively with Primary Care Team and other interdisciplinary team members Orders and interprets labs•tests•and diagnostic studies Prescribes pharmacologic and non-pharmacologic interventions Initiates referrals and consults to specialty services Provides education to patients and their families Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes Exemplifies the ICARE core values of-Integrity•Commitment•Advocacy•Respect•and Excellence Providers may be required to travel to the CBOC's or provide gap coverage as determined by the supervisor Provider will be required to be a designated women's provider and complete the mandatory VHA training VA offers a comprehensive total rewards package:
