The Medical Group provides a wide range of offerings, including primary care, specialty care, behavioral and mental health, and urgent care, both digitally as well as at physical locations in Massachusetts, New Hampshire, and Maine. The group also offers outpatient surgery and endoscopy, imaging, cardiac testing, and infusion. We share the commitment to delivering a coordinated and comprehensive experience across all locations, ensuring the appropriate level of care is available to every patient across our care delivery sites. Our Pease Family Medicine practice provides comprehensive care for patients of all ages, including physical exams, routine screenings, and preventive services aimed at identifying and managing health risks early. The team diagnoses and treats both acute and chronic conditions while promoting overall health, disease prevention, and long-term well-being. In addition to traditional primary care visits, the practice offers same-day sick appointments. Providers work autonomously at the top of their license within a collaborative, team-based environment. We are seeking a full-time, 40-hour Nurse Practitioner to join our Family Medicine team at 67 Corporate Drive in Portsmouth, NH. The office is open Monday through Friday, 8:00 AM to 5:00 PM. The schedule includes 32 hours of direct patient care and 8 hours of administrative time. Providers typically see patients in the office four days per week, with one administrative day that can be completed remotely. This provider will manage a panel of approximately 1,200 patients. Job Summary The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. Yes Essential Functions- Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
- Performs complete histories and physical examinations.
- Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
- Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
- Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
- Performs bedside procedures as are appropriate to the patient population.
- Orders medications and writes prescriptions in accordance with organizational and regulatory policies and procedures.
